Island Shore Physical Therapy Functional Institution
Once our athletes complete their individual clinical rehab with us and are cleared for return to sport by their referring doctor, they can continue and progress their specialized treatment program at The Island Shore Physical Therapy Functional Institute at Pro Game Athletics located in Bay Shore. At Pro Game, a 14,000 sq ft sports specific facility, athletes have the ability to run, throw, catch and hit in a safe and structured environment. This will prepare them to return to the field, the mound or the court ready to resume their given sport with the optimal functional skills, ready to perform without restrictions.

To make you first appointment as successful as possible, we ask that you bring
-Government issued Photo ID
-A form of payment (cash,debit or HSA card) for your copay
-Insurance card
-Physician Prescription
-Medication list (if applicable)
The cost of treatment is determined by your insurance carrier , your needs and diagnosis. Our front office team will verify your benefits prior to starting treatment in our office. This allows you as the patient to ensure that your care is affordable and that you are aware of any out-of-pocket costs that might be associated with your care.
Our office will provide you with a complimentary copy of your records. We do ask for 24-48 hours’ notice to allow us time to process this request. If the patient is not picking up the records, we will need a HIPPA on file and a valid ID that matches the name on the HIPPA. This is to ensure that the records are being given to the correct person.
